Job Description
Admit clients to program and complete initial assessment to ensure appropriate services will be provided based on identified needs. Perform admission procedures completely and accurately by SOC date as observed by record review and supervision. Arrange for initial visit and review consent forms with client and obtain signature. Complete assessment of client/family needs within two (2) working days of SOC as observed by record review and supervision. Complete physical review of systems, assess psychosocial and functional status, assess areas of vulnerability, teaching needs and environment. Plan for provision of care to ensure coordinated, medically reasonable and necessary services are provided. Initial medical Plan of Care for each client based on assessed needs and according to physician orders within two (2) working days of SOC as observed by records review and supervision. Develop the nursing care plan for each client based on assessed needs and areas of vulnerability within two (2) working days of SOC as observed by records review and supervision. Collaborate with interdisciplinary team members when needs are identified within two (2) working days of SOC as observed by record review and at least monthly thereafter as observed by participation of apropriate disciplines in case conferences. Assign duties to Staff Nurse, LPN, Home Health Aide, and Homemaker when needs are identified and orient Home Health Aide and Homemaker to home setting and plan of care within two (2) working days of date needs identified as observed by record review and supervision. Make referral to ancillary services and community resources based on identified needs within two (2) working days of date needs identified as observed by record review and supervision. Implement plan of care to ensure the client achieves established goals. Provide direct skilled nursing care, safely and effectively, according to established policies and procedures as observed by record review, supervision, and client's achievement of goals. Instruct client/family of the disease process, treatment techniques, medication management, diet, health maintenance and other identified needs according to teaching protocols as observed by record review and supervision, and client response to teaching. Supervise LPN, Home Health Aide, and Homemaker activities according to Medicare and JCAHO requirements, and as identified as observed by record review and supervision. Obtain necessary DME and other equipment/supplies needed to provide services in the home within two (2) working days of date needs identified as observed by record review and supervision. Adhere to Medicare, JCAHO, and State LIcensure standards to ensure compliance with regulatory bodies. Evaluate care provied to ensure positive client outcome is achieved. Assess and document client symptoms, response to interventions, and progress toward goals on each visit as observed by record review and supervision. Update/revise medical plan of care and nursing care plans when changes in client condition warrant and at least every two months as observed by record review and supervision. Discharge client from care when goals are met and/or when client no longer qualifies for service as observed by record review and supervision. Participate in quality assurance record review activities per management and as observed by record review activities and supervision. Provide input into Staff nurse, LPN, Home Health Aide, and supervision of the employee by date appraisal is due as observed by supervision. Communicate with client/family, team members, and other health care professional to ensure coordination of services. Document, according to established charting format and third party payer criteria in a clear, concise and legible manner, within two (2) working days of occurrence as observed by record review and supervision. Communicate with physician when client's condition warrants as observed by record review and supervision. Communicate plan of care verbally and in writing when referrals are made to other resources within two (2) working days as observed by record review and supervision. Participate in Hospice Team Meetings and Home Health Care Conferences when appropriate as observed by record review and supervision. Maintain departmental productivity standards to ensure services are provided cost effectively within budgetary guidelines, and ensure 24 hours availability of service. Complete work on a timely basis according to established productivity standards as observed by supervision. Seek and recommend alternatives to save time and/or operate more cost-effectively. Provide On-Call support for Home Health, Hospice as scheduled. Demonstrate flexibility and an availability with regard to on-call responsibilities. Follow established hospital and departmental precautions and procedures in the performance of all job duties to ensure a safe work environment for self and others. Demonstrate the competent use of proper body mechanics in all functions which require the movement of an object and understanding of universal safety precautions by not 9incurring more than one avoidable injury per evaluation period as evidenced by documented injury reports. Demonstrate a concern for hygiene and a clean work area and practice proper infection control techniques (handwashing) with no variances per evaluation period as observed by or reported to supervisor/department head. Demonstrate an understanding of emergency procedures as evidenced by documented review of policies and procedures with no variances. Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a profesional, responsive and courteous hospital environment at Immanuel St. Joseph`s. Interact with all others in a considerate, helpful, and courteous manner as established by departmental practice and procedure, patient survey responses and observed by supervisor/department head and peers. Present neat appearance in appropriate attire and identification as required by position and department with no more than two noted variances per review period. Remain calm during stressful situations and vent emotions and frustrations at appropriate times and places with no more than one documented variance per review period. Maintain confidentiality of designated hospital and patient information at all times as observed by supervisor/department head, peers and computer audit reports. Participate as a team member by performing additional assignments to meet organizational goals as noted and observed by supervisor/departmental head and peers. Participate in self-development activites to enhance job performance through formal means or less structured mean. Demonstrate interest in improving job performance through formal means, as evidenced by documentation on education log. Demonstrate interest in improving job performance through less structured means as observed by supervisor/department head.

Basic Qualifications:
Experience : A minimum of one year previous experience as Registered Nurse or Licensed Practical Nurse required. Education: Associate of Art or Bachelor of Science in Nursing required. Public Health Nurse certification preferred.

License or Certification:
Licensure & Certification: Registered Nursing (RN) License in the state of Minnesota required. Public Health Nurse (PHN) Certification preferred.

Schedule Details: Rotating days with rotating on call and weekend responsibilities.
Weekend Schedule: Requires rotating weekend and holiday coverage
