A Local Government body, well situated in the South East of London, is searching for a Qualified Care Manager to join their Older Persons division.
Duties Include:
? Assessing the needs of individuals and their carers
? Formulating and implementing care plans as appropriate
? Providing counselling and continuing support to users and carers where appropriate
? Monitoring, reviewing and evaluating services in order to ensure that they are provided effectively and in accordance with needThe successful candidate will be a car driver and have experience of safeguarding as well as care management for older people. An excellent grasp of current legislation is essential as well as skills in assessment and judgement as to when specialist input is needed.
