Main Purpose of the job: To assess the needs and wishes of service users and where appropriate, to incorporate these assessments into individual care plans or support service packages.To be responsible for ensuring the delivery of high quality service to the following service user groups, all of whom will have asylum status or fall within another immigration category: a. Unaccompanied minors who have "looked after" status. b. Unaccompanied minors who are Care Leavers, or who are Children in Need. c. Single adults, families, and adults with special needs. 1. To maintain accurate and full computer and paper records of interventions with service users including observations, action initiated and identified unmet need. To complete statutory and administrative forms where necessary and to provide advice, assistance, evidence and written assessments in court proceedings. To participate in staff meetings as required. To attend and participate in care/case reviews/conference as required. 2. To use regular supervision to ensure an effective service users and carers and further professional development. To undertake student supervision in accordance with departmental policies and priorities. To participate in duty systems as required. To be adhere to appropriate financial procedures. To undertake to some work outside office hours as necessary. 3. Assessment and Service Provision To undertake the assessment of the needs and wishes of service users and their carers and ensure their incorporation into individual care plans with particular reference to discriminated against groups. To participate in multi-disciplinary assessments and to undertake care manager and key worker/service provider functions where appropriate. To participate in the costing, co-ordination, monitoring and review of care plans. To meet service standards as agreed with the Team Manager and in conjunction with the Quality Assurance and Development Service. To provide a social work service as agreed in care plans and in accordance with departmental policies and priorities. 4. Service Development To contribute to new service developments and to ensure that new initiatives are responsive to the needs of discriminated against groups. To participate in the development and implementation of quality service standards in conjunction with the Quality Assurance and Development Service. To feed information in respect of unmet need, taking account of discriminated against groups into the relevant planning and review systems. To participate in the review and development of policies, practices and service in accordance with changes in legislation and community needs with reference to discriminated against groups. 5. To provide assessments that facilitate decision making by appropriate line managers (dependent on the level of expertise and training). |
