Working within the Adults with Learning Disabilities team, this Qualified Social Worker will have the following responsibilities:

- Provide a responsive person-centred social work service within a multi-disciplinary team.
- Developing care plans and managing caseloads.
- Monitoring and reviewing packages of support and maintaining up to date files.
- Use appropriate methods of assessment and intervention within relevant statutory and departmental requirements. This will include day services and supported accommodation provision.

The successful Qualified Social Worker will be GSCC registered, have a current CRB and 2 years post qualification experience.
